Midnight yesterday,I received a QQ message to inform me that an email had been sent to my mailbox from my tutor,Mr. Zhou. The QQ message also told me that my paper has been accepted. I shall carefully revise and submit it again soon. The contents of the email are as follows.

Comments to the Author

The authors provide a summary of clinical, laboratory, virologic, and histologic followup of 325 patients with chronic HCV infection who achieved an SVR24 after treatment with a PEG IFN+Ribaviran regimen. The data extend the understanding of the benefit of SVR and confirm that a minority of treated people recover normal liver tissue. The data also again point to the importance of initiating treatment prior to the development of cirrhosis.

Suggestions to improve the manuscript:

There are some grammatical issues throughout the text which should be addressed.

Methods:

Page 5: Please clarify whether or not the 2 pathologists blindly rescored all the pre-treatment as well as post-treatment biopsies separately, i.e. each separate biopsy was scored independently (and not as a pair). This is important as it is the soundest way to determine whether or not a histologic improvement occurred. See the methods in George et al, Hepatology, 2009 (Ref. 28).

Results:

1. The text on pages 7-8 about virologic relapse contradicts the information in Figure 1. The text states that of 29 people with a post SVR detectable HCV RNA, only 2 were transient where 27 were repeatedly positive. The Figure shows that 23 were transient and only 6 were sustained. These are contradictory.

2. Table 1: please show the normal ranges for ALT, AST, GGT, ALP, and AFP for ease of interpretation.

3. Table 2: please clarify when the first post-therapy sample was collected. At the 24Week post-treatment visit? Please also clarify or give a range for the collection of the long term followup sample.

4. For the patient who had sustained detectable HCV RNA after SVR24, how did you determine that these were relapses and not reinfections? Were they the same genotype as the pre-tx infection? Same or very similar viral sequences?

5. Page 10: presumably the patient who died of complications of cirrhosis during long term followup was related to hepatitis C ? (Lines 36-44).

6. Please state in the Results how many patients developed cirrhosis on long term followup. The abstract states 5, but this is not mentioned in the Results.

Discussion:

Page 13: Propose that the sentence at the end of paragraph 2 should be: “However, our data confirmed that the risk of late development of HCC after SVR in patients with pre-treatment cirrhosis is a real problem,..”

Suggest the authors state in the Results or Discussion that 37 patients had normal or nearly normal livers on long term followup biopsy. This is stated in the abstract, and it is an important point.
